I keep mine folded in a small plastic wallet approx 9x13 and there's no problem with scanning through the plastic.

The Pfizer vaccine no longer has to be kept in the low freezing temperature it was at the start of vaccinating, so if your doc has a vaccinating session, you should be fine.

I booked at our local pharmacy. I was able to choose the day and the time.

I think that's favourite for us, much closer than the doc and I can walk in and ask face to face. 6 calls to the doctor in 2 days without getting through to a human, and a 30 km roundtrip only to be turned away because the receptionist isn't full time is not an option. All I have to do now is wait for the Pharmacist to come back from holiday.

I keep mine in my TousAntiCovid wallet.
No dog ears there.

Try copying the document & printing down to A6. It makes it easier to carry around.
I've got it on my phone but, in case I don't have my phone handy, also carry the folded A6 version in my CV 'wallet' which is always in my 'proper' wallet which in turn is always with me when I'm out.
I've asked them who control these things to 'try' my A6 version & it works just fine.

My little plastic pochette works very well and it's a tad smaller than A6. There are many occasions when I need just ID, driving licence, PS and a couple of notes so I have a small purse I can attach to a belt (I don't use a smartphone).

It's too early to book on Doctolib for mid-January so will have to keep trying every week.
However, we don't know yet whether the vaccination centre in Saint Malo will remain open after December. With the number of people coming in increasing fairly dramatically, we're hoping it will.
