Are there forms available to fill in for a DNR? Notarised, sworn, signed by the President of France? Anybody gone through getting one, svp?

Do Not Resuscitate
My (father's) experience was the hospital put a DNR on his file - without consulting him - during his penultimate illness.
His partner spotted it - and it was very much not what he wanted. So it may be other medical staff will also ignore a patient's wishes - however expressed?

I filled out one when I was going through cancer treatment several years ago - I got it from my Specialist and I think it's noted on my medical records which are all linked together - whether or not my family can override it or not I don't know, and it never occurred to me that the medical staff might not take any notice of it.

Yes. He had terminal cancer - but someone obviously decided if his heart stopped first then they would not restart it.

Having gone through this recently with the death of my wife with terminal cancer. Near the end we worked with TERDASP for the palliatif care. This includes no resuscitation.
My wife had decided many years ago to donate her body to medical science and had it all sorted out.
