Sorting out my S1
as @suein56 says - do not send the original - it can disappear to no one knows where and that's a strong possibility! If you have a good colour printer or know someone that has then you can't tell the difference between the original and the copy - I know cos that's just what I've done but don't tell anyone

From your link, following the link for Affiliation / Immatriculation on the LHS, this is the form for requesting affiliation, but the note at the top indicates that it is NOT to be used in your case.

The note reads "Si vous relevez du régime de sécurité sociale d'un autre pays, par exemple en tant que travailleur détaché depuis l'étranger ou retraité, vous ne devez pas remplir ce formulaire. Pour exercer vos droits et ceux des membres de votre famille, vous devez fournir à votre caisse d'assurance maladie un formulaire S1 "Inscription dans l'Etat de résidence" ou un document équivalent pour les pays hors espace européen."
From which it seems you simply send them your S1, maybe with a covering letter stating your current situation.

Sorting out my S1
It’s my Birthday today. I rang the NHS again about my S1. 45 minute wait.
Tried to fob me off by saying they hadn’t had any info from the DWP.
I asked how I had a Case number. Oh let me look again he says. Oh I didn’t see that .
Hmmmm. Anyway after a bit of a wait while he ‘contacted the department’ he assured me I would be sent an email with an S1. Total 1h 3 mins on phone.
Email has arrived saying the S1 has been posted and if I haven’t heard within 15 days(same time span everytime I’ve called) to contact them again.
I started this painful process in April. Pension application ‘approved’ in May
S1……………….
I see my surgeon on 6/9.

Coincidence, it's mine too. In theory, I've got a couple of years to go before being honoured with my pension, Do you think I'd best start applying now?

Sorting out my S1
Today I have received a secure email with a copy of my S1. I’ll log on to my computer after thunderstorms have gone and try to download it. CPAM may not be keen to accept a copy but I’ll give it a try.

Do you mean a copy from DWP? Mine was the original two S1s sent by post from DWP, I made two copies and posted them off to CPAM with copy of CV and passport. I have also started to receive my state pension

Sorting out my S1
They are sending my S1 by snail mail and they have also sent me an e-version. I haven’t opened it yet. Will look at it later on. I’m hoping CPAM Agen will accept a paper copy of the e-version as time is tight for me. Bearing in mind DWP don’t issue S1’s any more anything could happen, plus a postal strike and rail strikes will disrupt things this month.

I logged into the secure system and found the email BUT no attachment. I’ve contacted them by secure email and ordinary email. No response yet.

And WOW I’ve just received by post two copies of my S1. That was super quick!
Now to make an appointment at CPAM Agen to get them recorded in the system. After I’ve made copies.

So pleased that at least the first part is sorted. Good luck with CPAM, our one took quite a while to sort things but at least you should get an attestation.
